New Victorian resources to support timely, coordinated and affirming care
A new suite of free, practical resources is now available to help health professionals and community organisations strengthen care and referral pathways for trans, non-binary and gender diverse children and young people across Victoria.
Co–designed by Transcend Australia in partnership with The Royal Children’s Hospital Melbourne, Monash Health, Orygen and Transgender Victoria, the resources respond to barriers identified through statewide consultation with young people, families, and health professionals in 2023.
Participants consistently identified unclear referral pathways, long waiting lists and limited awareness of available services and care pathways as barriers to timely, affirming care. Health professionals also highlighted the need for practical guidance to support appropriate referrals and improve coordination across the health system.
The resource suite includes:
- Health Pathways for Trans & Gender Diverse People in Victoria – A practical guide to referral pathways, available services and best practice for supporting trans and gender diverse young people. [Link]
- TGD Health Services – A Victorian guide to gender affirming care, peer and community services across Victoria to support appropriate referrals. [Link]
- Best Practice Data Collection Guide – Practical guidance for collecting client information and referral data in a respectful and inclusive way. [Link]
